SPA Referral Information

Weston General Hospital

ED (Orthopaedic)
1. Referral Received
2. Contact the Site Manager to discuss availability
3. Contact the Surgical or Orthopaedic SHO dependant on speciality via Bleep
4. Arrange Transport if required via Great Western Ambulance Service Control Room
5. Confirm arrangements with the patient and or family
6. Fax the referral information through to ED
7. Call ED to confirm receipt

EAU (Gynae and Surgical)

1. Referral Received
2. Contact the Site Manager to discuss availability
3. Contact the Medical / Surgical SHO dependant on speciality via Bleep
4. Contact the EAU Senior Nurse with patient details to advise how the patient will arrive
5. Arrange Transport if required via Great Western Ambulance Service Control Room
6. Confirm arrangements with the patient and or family
7. Fax the referral information through to EAU
8. Call EAU to confirm receipt

Assessment and Treatment Centre (ATC)
1. Referral Received
2. Contact Senior Nurse or On Duty Doctor at ATC to discuss referral
3. Fax the referral information to ATC
4. Call ATC to confirm receipt
5. Arrange Transport if required via Great Western Ambulance Service Control Room
6. Confirm arrangements with the patient and or family

Integrated Community Response Team (ICRT) – Also cover Community Physiotherapy, Occupational Therapy, Safe haven Beds and Falls Co-ordinator

1. Referral Received
2. Contact ICRT to discuss referral
3. Fax Referral information to ICRT
4. Call ICRT to conform receipt
5. Confirm arrangements with the patient and or family



Bristol Royal Infirmary, Bristol Eye Hospital, Southmead Hospital and Frenchay Hospital  - in all cases contact the Clinical Site Team to make the referral.
