Self Care includes both self care and self management.
- Self care is about people learning to look after themselves
in a healthy way.
- Self-management is about people coping better with their ill
health in a way that helps avoid difficulty and makes the most of what
they have. It includes managing or limiting the way conditions affect
individuals’ lives. This should lead to feeling happier and more
fulfilled despite their condition.
Supporting self care is about working in partnership. It is a two-way process
that includes discussion and making decisions. Both the person and the care
professional contribute to making a plan that gives the best possible result for
- Information Prescriptions – instead of receiving a
prescription for a drug you are given a prescription for information.
Your Information Prescription will help you find the information you
need so you feel more confident and better informed to manage your
health. They are becoming nationally recognised as a source of
information on services and care. In future they will be part of the
care process. North Somerset libraries hold a number of recommended
books that may be prescribed as well as being a place where you can seek
advice to find more information.

- Personalised Care Plan – a plan that is made just for you
that records the results of the discussion between you and your
professional and what you have agreed as your goals. It will also
include ways you can achieve those goals. It could be written and stored
on a computer, or written on paper in whichever form you prefer.
